Say whether the problem is constant or comes and goes. If another company has already assessed it, have the written findings available so you can explain what has been checked.

- The service address and a phone number where you can be reached.
- When the issue first appeared and whether it has happened before.
- Which fixtures, rooms, or equipment are affected.
- Equipment make and model, if already accessible without opening panels.
- Any access restrictions, parking instructions, or appointment constraints.
